The object of the contract is the execution of the advanced home telecare service, in accordance with the conditions established in the Technical Prescription Document. Individualized care will be based on the person-centered model that allows users, through a specific communications system, to have a permanent and uninterrupted care service, which acts not only in emergency situations or social needs, but also in a preventive, proactive and predictive manner based on established protocols and programs.
This service includes complementary technological support, inside or outside the home, as well as the interconnection with the analytical management platform under development by the Community of Madrid, through processes and action protocols based on the situation of need for care detected. To do this, it has professionals with specialized training and technological solutions to provide an adequate and personalized response to the needs of users.
