Procurement plan for Sale and delivery of a tissue homogenizer for the faculty of biology, nature conservation and sustainable development
1. The subject of the contract is: Sale and delivery of a tissue homogenizer for the Faculty of Biology, Nature Conservation, and Sustainable Development. The Ordering Party does not allow partial bids. Reasons for not dividing the contract into lots: included in the ToR. 2. A detailed description and method of contract execution are included in the description of the subject of the contract (OOP), constituting Appendix No. 1.1 to the ToR. 3. The contract may be awarded to Contractors who meet the following conditions: 1) the ability to engage in business: the Ordering Party does not impose any requirement in this regard. 2) the authorization to conduct specific business or professional activities, if so required by separate regulations: the Ordering Party does not impose any requirement in this regard. 3) economic or financial standing: the Ordering Party does not impose any requirement in this regard. 4) technical or professional capacity: the Ordering Party does not impose any requirement in this regard. 4. The Contractor is required to attach the following attachments to the offer (the Offer Form, completed and signed by the Contractor - Appendix 1 to the Technical Specifications): 1) A signed description of the offered subject of the order – prepared by the Contractor (including the manufacturer, model, technical parameters, etc.). The description of the subject of the order offered by the Contractor cannot be supplemented because it constitutes the content of the offer by specifying the features hidden under the indicated name of the product/device offered by the Contractor. Due to the fact that providing the model or catalog number of the product/device itself does not always allow for its identification, the Ordering Party, in addition to the requirement to provide the name characterizing the device, requires the actual technical parameters of the offered product/device. 2) A declaration, current as of the date of submission of offers, that the Contractor is not subject to exclusion and meets the conditions for participation in the procedure. The Contractor shall submit this declaration in the form of the European Single Procurement Document (ESPD), constituting Annex No. 2 to Commission Implementing Regulation (EU) 2016/7 of 5 January 2016 establishing a standard form for the European Single Procurement Document. The information contained in the ESPD constitutes preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the procedure. The Contracting Authority informs that the instructions for completing the ESPD and an editable version of the ESPD form can be found at: https://www.uzp.gov.pl/e-uslugi/jedz ; https://www.uzp.gov.pl/baza-wiedzy/prawo-zamowien-publicznych-regulacje/prawo-krajowe/jednolity-europejski-dokument-zamowienia . The "ESPD" template constitutes Annex No. 2 to the ToR. In the case of Contractors jointly applying for a contract, the declaration on the single document form must be submitted by each of the Contractors jointly applying for the contract. This declaration is intended to confirm the absence of grounds for exclusion within the scope specified in these ToR. The Contractor and each Consortium participant shall submit their own European Single Procurement Documents. In the case of a Contractor that relies on the capacities of other entities, the Contractor shall also submit an ESPD for each of these entities, confirming the absence of grounds for exclusion with respect to them and confirming compliance with the conditions for participation in the procedure to the extent to which it relies on their resources, signed by persons authorized to represent a given entity. In Part IV of the ESPD, the Ordering Party requests only a general declaration regarding all qualification criteria (section a), without completing individual Sections A, B, C and D; Part V (Limitation of the number of eligible candidates) should be left blank. 3) Declarations by the Contractor/Contractor jointly applying for the award of the contract regarding the grounds for exclusion under Article 5k of Regulation 833/2014 and Article 7 paragraph 1 of the Act on special measures to counteract support for aggression against Ukraine and to protect national security, submitted pursuant to Article 125 section 1 of the Public Procurement Law, the template constitutes Appendix No. 5 to the ToR. 5) Power of attorney to sign the offer (if applicable). 6) Commitment of another entity and declaration in the form of a Single European Procurement Document (ESPD), referred to in Chapter X (if applicable).
