REQUEST FOR QUOTATION The Bayambang Water District BayWaD, hereinafter referred to as the “Purchaser”, through its Bids and Awards Committee BAC, invites interested suppliers to submit price quotations for the project: “SUPPLY AND DELIVERY OF 60KVA GENERATOR SET FOR BRGY. Duly Notarized Omnibus Sworn Statement OSS________________________________________ DELIVERY TERMS: • Delivery Period: Within fifteen 15 calendar days from receipt of Purchase Order PO. • Delivery Hours: Monday to Friday , 8:00 AM to 5:00 PM • Delivery Site: BayWad Warehouse, Brgy. Sapang, Bayambang, Pangasinan • Pre-delivery Notice: Supplier must notify Purchaser at least two 2 days before delivery • Return Policy: Purchaser reserves the right to reject or return defective items ________________________________________ VALIDITY OF OFFER: Price quotation shall remain valid for thirty 30 calendar days from the deadline of submission. ________________________________________ DISCLAIMER CLAUSE: Bayambang Water District reserves the right to accept or reject any quotation, and to annul the procurement process or reject all quotations at any time before contract award, without incurring any liability to the affected supplier/s.
