The subject of the order is the supply of consumables and accessories used in the Operating Blocks of the University Clinical Hospital of Fryderyk Chopin in Rzeszów, including the supply of disposable fibers for the Operating Block of the Urology and Oncology Urology Clinic along with the lease of a thulium laser for urological procedures. Tenders are invited for Deliveries of consumables and accessories used in operating theaters. 1. the subject of the order is the supply of consumables and accessories used in the operating theaters of the university clinical hospital. fryderyk chopin in rzeszów. 2. a detailed description of the items of the order, estimated quantity and requirements are presented in the price and offer forms for parts no. 1÷23 (appendices no. 1 to the swz), price and offer forms for part no. 20 (appendices no. 2 and 3 to the swz) and the list of parameters required for part no. 20 (appendix no. 4 to the swz), which are an integral part of the swz. 3. the scope of the order includes the delivery of the items of the order in the quantities specified in the price and offer forms for parts no. 1÷23 (appendices no. 1 to the swz) and in the price and offer form for part no. 20 (appendix no. 2 to the swz), which are an integral part of the swz, for a period of 24 months from the date of conclusion of the contract. 4. the ordering party does not allow the submission of equivalent offers in parts no. 1 to no. 10, in part no. 12 and in parts no. 14 to no. 23. 5. the ordering party allows equivalent solutions in parts no. 11 and 13 as described in the tor. 6. order completion deadline: 1). delivery of the items of the order for 24 months from the date of conclusion of the contract (does not apply to device leasing). 2). the ordering party allows for the possibility of changing the contract validity period in the event of failure to complete the entire subject of the contract within the contract period due to the execution of lower orders. changing the contract requires an annex, however, the ordering party unilaterally notifies the contractor about the proposed extension of the contract before the expiry of the period for which it was concluded (does not apply to lease).
