Procurement and Installation of Statistical Analysis Software for Reasearch Data Center- RDE OMSC RFQ 26-025
The OCCIDENTAL MINDORO STATE COLLEGE OMSC through its Bids and Awards Committee BAC, intends to procure the Advanced Statistical Analysis Software for Research Data Center- RDE OMSC RFQ 26-025 with an Approved Budget for the Contract ABC of Eight Hundred Twenty-Six Thousand Three Hundred Fifty Pesos Php826,350.00 through Section 34 Small Value Procurement of the IRR of RA No. 12009 or the NEW GOVERNMENT PROCUREMENT ACT. Award shall be made on: [ ] per item [?] per lot. Award shall be made to the lowest evaluated quotation that complies with the minimum technical specifications and other terms and conditions stated herein. Please submit your duly signed Quotation Forms Technical Specification and Financial Requirement including the Terms and Conditions Annex “A” together with the following mandatory requirements on or before 2:00 PM of June 22, 2026: a. Valid Mayor’s/Business Permit b. Professional License / Curriculum Vitae for Consulting Services c. Income Tax Returns¹ for ABCs above Php500,000.00 d. Notarized Omnibus Sworn Statement GPPB-Prescribed Form and if applicable, Original Notarized Secretary’s Certificate in case of a corporation, partnership, or cooperative; or Original Special Power of Attorney of all members of the joint venture giving full power and authority to its officer to sign the OSS and do acts to represent the Bidder for ABCs above Php50,000.00 We highly encouraged interested suppliers to use the Quotation Form provided to minimize errors or omissions of the required mandatory provisions. If the quotation form will be reproduced/retyped, please ensure that all mandatory requirements/provisions are included. Missing requirements/provisions and non-submission of the terms and conditions shall render the submission ineligible or disqualified. The Head of the Procuring Entity HoPE of the OMSC reserves the right to reject any and all quotations, declare a failure of procurement, or not award the contract at any time prior to contract award in accordance with Section 70 of the of the IRR of RA No. 12009 or the NEW GOVERNMENT PROCUREMENT ACT, without thereby incurring any liability to the affected bidder or bidders.
